Day Centre, Canton
02/12/2024 – 3 months initially
Monday to Friday, 9am – 5pm, 37 hrs/week
Our client are seeking a Social Work Assistant to join a Supported Living, Learning Disabilities Team based in Canton.
- Direct work with adults with learning disabilities to enable them to live closer to home
- Assisting adults with learning disabilities to resolve major difficulties in their lives and to live as independently as possible
- Undertake wellbeing and risk assessments while considering outcomes
- Undertake non-complex capacity assessments
- Make recommendations regarding packages of care to meet identified needs, considering all available resources
- Develop, implement and review care packages
- Report any cases that become complex and require social work input
- Work with other agencies such as health, housing, police where needed
- Being part of planning and facilitating moves for individuals
- Carrying out Monitoring Visits to Supported Living Schemes
- Assisting with the overall Monitoring of the Supported Living Contact
- Keep and maintain accurate records
- Attend meetings, reviews and conferences where necessary
- UK Driving Licence, access to a vehicle, business insurance
- Experience of working with individuals with learning disabilities and/ or Supported Living
- Experience in assessments and care packages
- IT Skills and experience of using SharePoint and Excel Spreadsheets are strongly desired
- Excellent written and verbal communication skills
- Enhanced Adult DBS registered to the update service
