Brokerage Officer – Nottinghamshire

Salary: £ 13.41 Per Hour

Full Time (37 hours per week)

Role Purpose:

To co-ordinate and support the sourcing and arrangement of home based care services under the direction of Community Partnership Officers.

Responsibilities:

• Responding to requests from commissioners for home based care services for users of managed services
• Liaising with providers to source and commence home based care services that have been commissioned for individuals; following established processes and procedures
• Liaising with providers to enact changes to home based care packages, as instructed by commissioning workers
• Keeping in regular contact with care providers to maintain an overview of service capacity; liaising with Community Partnership Officers regarding concerns or issues
• Recording and updating social care records on Mosaic and other information systems
• Supporting and assist the Community Partnership team in maintaining and analysing management information, qualitative and quantitative data relating to home based care
• Acting as a point of contact for day to day queries from service users/carers and operational staff and to contribute to initial problem solving responses to complaints or concerns in respect of service provision
• Communicating with service users regarding changes in packages of care, including changes of provider
• Undertaking telephone customer satisfaction surveys in respect of home based care services; using established processes
• Supporting the Community Partnership team to maintain and ensure effective communications between providers and service users
