Are you a passionate, dedicated Care Coordinator looking for your next role? 4Recruitment Services are recruiting a Care Coordinator to work in Kingston.

The Care Coordinator will support the Reablement Manager to deliver a high quality Reablement service, to meet the needs of service users. This involves working with users and careers in setting goals and conducting goal reviews, carrying out Reablement tasks, managing and recording the progress and, when required, supporting the Reablement Manager or Deputy Reablement Manager in running the service.

Pay Rates: £16.28 - £20.21 per hour

Responsibilities of a Care Coordinator: (Full Job Description Available)

  • To carry out risk assessments and disseminate these to relevant staff
  • To assess users for basic equipment and issue, if required.
  • To set out Reablement goals and produce action plans with users and their carers
  • To co-ordinate the work of Home Care Enablers
  • Carry out duties in accordance with the Council’s Health and Safety Policy and relevant Health and Safety legislation, including moving and handling, and risk assessment
  • Take responsibility for reviewing service users long term needs, referring on to and liaising with Social Services teams in an effective and timely manner.

 Essential Requirements:

  • Educated to degree level or equivalent
  • Experience of working within Health or Social Care 
  • Excellent understanding of concept of ‘Reablement’/ ‘enablement’
  • Ability to carry out quality checks assessing the performance of Home Carers.
  • Right to Live and Work in the UK.
  • Current DBS on the Update Service.
  • Driver with access to use of vehicle
