Care Management, CARE LEAVER Social Worker Isle of Man
£35 per hour
Plus help towards travel/accommodation.
Although this role is within the Care Management team, the predominant responsibilities will be for children and young people 15+ who are planning transition to leaving care and semi independence.
This role will be responsible for taking the lead on pathway plans and supporting these older young people for next steps.
