4Recruitment Services are seeking to recruit Community  support workers to help support the Discharge 2 Assess (D2A) review work.  This role is to complete care act assessments/reviews. Most assessments will need to be completed in person, however the client will allow for workers to be home based and therefore will allow some hybrid way of working. It is important such candidates will need to be able to travel easily  around  the Leicestershire area.

DUTIES AND RESPONSIBILITIES INCLUDE:

  • Ensure high quality responses to all citizens eligible for social care services by;

-       providing appropriate responses or referrals within required timescales,  organisational, policy and legislative requirements including the reimbursement legislation and eligibility guidelines

-       that concerns and complaints are responded to and are addressed by making appropriate changes to existing practices

  • Promote independence and facilitate solutions through self-directed support, providing practical, person-centred assistance to citizens, their family/carer and their support network to:

-       assess and review their own risks and needs and determine the allocated personal budget

-       design and cost a mutually-agreed best value support plan to meet agreed outcomes, exploring all options including cash or managed personal budgets

-       liaising and negotiating with local providers, support networks, and other resources as appropriate to deliver better outcomes

-       access, use and review services, including the effective use of cash payments and the safe use of equipment and adaptations in compliance with relevant legislation, policy and guidelines

-       arrange, implement and review support plans if requested by the citizen, and providing intensive support if they require it to start activities or assess potential services

  • Identify and manage potential safeguarding, mental capacity, continuing health care and other high risk situations where citizens’ safety or liberty may be at risk, referring up as necessary so as to enable citizens to take risks whilst protecting them from abuse, managing the risks to staff and the Council
  • Refer to the line manager unresolved support and service issues, including

-       complex care and support arrangements

-       cases where it appears that outcomes to meet needs cannot be delivered by the personal budget allocated

  • Provide assessment, care planning and review functions for citizens who do not have personal budgets and/or where services are commissioned by the council, including review of services’ quality and achievement of outcomes. Making necessary adjustments to those arrangements in line with the changing needs of the individual, including wherever possible their personalised needs and wishes

ESSENTIAL REQUIREMENTS:

  • A good level of general education including GCSE English or equivalent qualification
  • Relevant health/social care qualifications to NVQ Level 3 or equivalent relevant experience
  • Relevant experience (paid employment, training or voluntary sector) working with adults in a social care setting
  • Experience of work with other agencies within a fieldwork environment
  • Knowledge of the needs and issues of citizens and carers including ethnic minorities, disability and age issues
  • Knowledge and understanding of the need to undertake need and risk assessments
  • A good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector
  • Experience of working in a health or hospital setting/working with carers/undertaking assessments of need/ managing a caseload and monitoring and reviewing care packages would be desirable
  • Enhanced DBS
