Deputy Team Manager | Adults/Older People & Physical Disabilities Team | Milton Keynes

Contract –

We require an experienced Deputy Team Manager to work within an Adults/Older People & Physical Disabilities Team based within Milton Keynes. To be successful for this role, you are required to have a minimum of 3 years recent Team Manager experience of working within an Adults/Older People & Physical Disabilities Team.

You will be accountable for assessments and reviews of complex social care needs and provision of support for vulnerable adults to ensure they are safe and have good outcomes to promote independence. Working within the statutory legal framework, policy and guidance, determining when the threshold for statutory intervention has been reached in line with legislative and organisational standards within the Care Act 2014.

The successful Deputy Team Manager will have experience of:

  • Carrying out assessment's for Older Persons and their carers
  • Obtaining multi-disciplinary assessment of need
  • Care management
  • Designing intensive care packages
  • Carrying a caseload of service users
  • Make recommendations for short-term care, intermittent care and permanent residential care where appropriate
  • Self-directed support / personal budgets

To apply for this role you must have:

  • A relevant Social Work qualification & experience (BA/MA Social Work/DipSW or equivalent)
  • Experience of completing high quality Form F assessments and presenting reports to Panel
  • Older People & Physical Disabilities team experience
  • Be HCPC Registered
  • Be eligible to work within the UK

Salary is £33 per hour.
