General Community Nurse

4Healthcare are currently recruiting for Full-Time Band 5 Community Nurses (CNs) to work within Southampton for a minimum of 2 months.

The Community Nurse will be required to provide nursing care, working under the supervision of the District Nurse Team Lead (DNTL) to patients in a variety of community settings. They will undertake community nursing duties involving delivery of nursing services and holistic assessments of patient's health needs using a person-centred approach.

This includes providing quality nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care institutions. The post holder will also be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.

The ideal candidate for this role will be a Registered Nurse with current NMC registration. They will have gained recent clinical nursing experience and have had exposure to working with people with complex health needs. The ideal candidate will also have previous experience in the Nursing process, care planning and be fluent in record keeping. Candidates will be expected to work calmly and effectively in a fast-paced environment, as well as communicate well with other members of staff and patients.

• IV Trained
