Are you a passionate, dedicated Leaving Care Support Worker looking for your next role? 4Recruitment Services are recruiting a Leaving Care Support Worker to join their Leaving Care team based in Bolton. You will be required to assist young people aged 16 - 25 in their transition to adulthood.

Pay Rates: £19.40 per hour

Duration: 06/03/2023 - 18/08/2023

Hours: Monday to Friday, 08:45 - 17:00, 37 hrs/week - Hybrid

Leaving Care Support Worker role: Full JD Available

• Take responsibility on a day-to-day basis for your individual caseload
• Assist young people to build support networks and independent living skills
• Prepare, produce, implement and evaluate plans to address behaviour
• Assess needs and options to recommend a course of action
• Work with service users employing a range of skills and knowledge and contributing specialist skills in particular areas, such as Welfare Rights
• Liaise closely with Pathway Coordinators and other service providers
• Maintain appropriate records of services provided to service users
• Attend reviews, case conferences, pathway planning and meetings where appropriate

Essential Requirements:

• NVQ Level 3 Caring for Children and Young People or equivalent
• Driving Licence, access to a vehicle, business insurance
• Recent extensive experience working with young people leaving care
• Ability to undertake assessments and review programmes, including the provision of pathway plans
• Knowledge of issues facing young people leaving care or becoming homeless
• You Must Have Right to Live & Work in the UK
• Enhanced DBS Certificate registered to the update service
