Job Description
We are currently looking for a Qualified Social Worker - Community & Integrated Discharge Team to work with one of our clients based in Rotherham. You will be working for the council.

Pay rate up to £32.99ph
Job Purpose
This is an exciting opportunity for social workers to work in Rotherham in our Community Adult Social Work Teams and Integrated Discharge Team, which cover the North, South and Central geographical areas of Rotherham.
Our focus is to support people and their carers in their own community to build on their strengths, assets and abilities to maximise their independence and wherever possible self-manage. As a social worker in this service you will also work closely with health colleagues and other partners to enable people to meet their outcomes and lead good lives.
The successful candidates will have a strong knowledge and experience of working within the statutory legal frameworks governing adult social care and have experience of safeguarding adults.
As part of the team, which comprises of social workers and vocationally qualified assessment and review co-ordinators, you will work with adults with physical disabilities, learning disabilities, older adults and their carers, who are requesting assessment and support from the council.
