We are currently looking for a Qualified Social Worker - Multi-Disciplinary Team to work with one of our clients based in Bracknell. You will be working for the council.

Pay rate - £28.38ph

Job Purpose

To work as a Social Worker as part of a multi-disciplinary team, providing support to adults who are experiencing significant mental health issues. To work in collaboration with colleagues from health and social care to assess needs under the Care Act 2014 and to provide Care Co-ordination duties under CPA. To support the duty function within the service. The team works closely with colleagues in the NHS to ensure that needs are met holistically taking into consideration the view of the individual, their family and carers. The role is to coordinate the identification of those needs and develop support plans to meet those needs whilst maximising choice, control and independence. To ensure that the health social care needs of people are met, within the eligibility criteria of the department, by working in partnership with people who use the service, their carers, professionals and other agencies, and service providers. To support in delivering and maintaining high standards of performance ensuring continuous development and supporting people to fulfil their roles to maximum effectiveness within allocated resources.
