Job Description
4Recruitment Services are seeking Reablement Support Workers.
This job involves you visiting people in their own homes. Applicants must be car drivers as you will be required to travel across the Solihull Borough. This includes Rural Areas, Knowle, Dorridge, Hampton In Arden, Meridan, Balsall Common. Please note the travel time to get to each call from your first to last call is built into your programme of work. Successful applicants will be paid for the contracted hours they are employed to work and not just the hours they work.
The purpose of the role is:
- To engage, encourage and motivate people to achieve their maximum level of independence through an assessment of need and the development of a personalised reablement plan.
- To give practical help, care, guidance and support to all people in retaining as independent a lifestyle as possible and also promote continued independence in their local community
DUTIES AND RESPONSIBILITIES INCLUDE:
- Ensure safety of people using the service through dynamic assessment of risks.
- Motivate and support the person to maintain contact with family and friends, individuals and the community.
- Recognise improvements and or developments in the person’s condition, report to line management and document accordingly
- Make minor adjustments to equipment, including assistive technology, following consultation with professional members of the team and record any changes made
- Signpost and support people to access other services.
- Apply appropriate techniques to support speech and language difficulties.
- Undertake therapeutic activities i.e. improving and or maintaining mobility (indoor and outdoor) and the use of appropriate equipment supplied , building confidence, building stamina, practicing personal care, practicing transfers and daily living skills.
- Monitor health and well-being including skin condition, continence issues and referrals to appropriate resources.
- Identify and report safeguarding concerns in line with the Directorate’s policy and procedures.
- Improving and maintaining financial independence – assistance to access banks, shops and other community resources.
- Maintain accurate records and documentation in line with requirements.
- Engage and develop positive working relationships in order to effectively support people and carers involving advocates and other professionals where appropriate
- Undertake training and development activities and meetings to achieve a satisfactory level of competence in all duties performed, including any QCF level 2/3 competences required with a view to ensuring personal and organisational continuous improvement.
- To contribute to the day-to-day support and development of colleagues across Adult Social Care.
ESSENTIAL REQUIREMENTS INCLUDE:
- Current Driving License and able to use own vehicle for business purposes. Must have car business insurance.
- QCF Level 2 in Health & Social Care or equivalent OR be willing to work towards obtaining on appointment
- Strong numeracy and literacy skills to undertake tasks such as providing advice on budgeting and accurate record keeping.
- Experience of working and communicating with people who have care needs (in a home or work setting)
- Experience of supporting people with independent living skills / providing activities and support within the provision of personal care.
- Knowledge/life skills of Health & Safety in particular risk assessing situations / environments and being able to be responsive and report identified risks.
- An awareness of dietary requirements and nutrition in relation to individuals
- Effective verbal and written communication skills and excellent interpersonal skills.
- Ability to work unsupervised and as part of a team.
- Enhanced DBS check
