Job Description

Norfolk County Council - Reablement Support Worker

Location: Benjamin Court, Roughton Road, Cromer, NR27 0EU (Will involve travelling between Benjamin Court and Grays Fair Court, also travelling between homes in these council catchment areas).

Contract: Initial Contract to 31st January 2021 with possible further extensions.

Hours: AM Shifts, PM Shifts and Nights – Full Time Hours – Weekends may also need to be worked.

Payrate: £10.41 an Hour

PRINCIPAL ACCOUNTABILITIES:

To provide encouragement, assistance, advice and support, to help service users gain confidence with personal care tasks, manoeuvring when getting in and out of bed,

dressing and undressing, getting in and out of a chair or on an off the toilet; with the aim to enable the service user to achieve maximum independence to enable them to take part in the activities of everyday life.

This may include bathing, washing, continence needs and feeding. 

To assist service users with mobility difficulties as identified in the care plan, including moving and handling as identified in the Moving Handling Risk Assessment.

To be flexible in meeting the wishes/needs of service users within Departmental policies and procedures and the service user plan ensuring that service users’ rights to privacy, dignity, respect and confidentiality are observed at all times.

To accurately and legibly complete documentation (this may include electronic format) relating to the visit, including risk assessments and daily record sheets and report any changes in risk to the Care Co-ordinator

To observe, monitor and report to the Care Co-ordinator any significant adverse changes or improvements in the service user’s health, mood, behaviour and attitude or changes in circumstances and actively participate within the reviewing process

To report back to the Care Co-ordinator / management team also to feed information into the multi-disciplinary team where it is identified a person may have needs outside of commissioned services for example domiciliary care,

loneliness, information and advice to the service user and their family/carers to enable onward referrals to be made to support a person to live independently

To work as part of the multi-disciplinary team liaising with other professionals and services as necessary to provide the best possible care to service users.

To be reliable and consistent and to notify either the service user of any delays as soon as possible and to maintain regular contact with the management team if a change to the rota is identified.

To collect/order prescriptions from designated GP or dispensing chemist as necessary and to assist, if identified in the care/service user plan, with the medication on a day-to-day basis after Departmental training and in line with Departmental procedures

To participate in fire safety for people within the building through participating in fire drills and evacuation procedures in the event of a fire

To assist with or cash pensions and allowances, to pay rent and other household accounts, shopping and securing cash and valuable items using the Departmental procedure and in line with the care/service user plan.

To advise, assist with or undertake laundry requirements; the planning, preparation, cooking and/or serving of meals or in conjunction with personal care needs advice, assist or undertake hygiene cleaning as necessary

Where the service team and service user agree to the need for an ongoing care package, to actively participate with the transfer to the new support provider to ensure effective transfer of the support package

To attend and contribute to meetings and supervision/appraisal sessions as required and participate in Departmental training, demonstrating an ability to adopt a learning attitude.
