Social Work Assistant – Assessment and Case Management  Cardiff 13/01/2024 – 3 months initially Monday to Friday, 8.30am – 5pm, 37 hrs/week Support assessments of care and support needs within the hospital social work team. Arrange and monitor appropriate services in line with individual care plans. Complete care plan reviews and or reassessments. Provide advice and practical help. Speak with them about any issues they may be having or are worried about. Liaise with voluntary and statutory agencies. Participate in team meetings and duty rotas. Undertake training and self-development and to contribute to this process in others.
