Substance Use Recovery Worker Slough 20/01/2025 – 3 months initially Monday to Friday, 9am – 5pm, 37 hrs/week Effective case management of service users with substance use issues Carry out comprehensive substance use assessments including assessment of personal strengths and identification of any risk factors Formulate, monitor and review risk assessments and risk management plans that effectively safeguard and protect service users and families/carers from harm Develop person centred recovery plans using a strengths-based approach in partnership with service users and others involved in their support network Deliver and support harm reduction interventions, including needle exchange and access to physical health care including BBV and health screening Delivery of psychosocial treatment interventions in groups and on a 1:2:1 basis Supporting service users to access clinical interventions where required including engaging in clinical reviews En...
