Patient Discharge Officer

4Recruitment Services are seeking  a Patient Discharge Officer for our client located in Northampton. The post holder will be working in an integrated multi professional patient flow team and will be involved facilitating a safe and timely discharge for all patients on the ward.   The post holder will co-ordinate timely patient referrals to relevant community services and liaise directly both with patients and their relatives as required about discharge arrangements.
