Adult Social Worker - Continuing Healthcare Specialist

We are currently looking for a Adult Social Worker - Continuing Health Care to work with one of our clients based in Berkshire. You will be working for the council.

Temp contract 

Pay rates - £16.32ph

Key Responsibilities

• To make contact with providers/care homes/agencies to gain information and evidence to support the LA review process and ensure they are keeping significant records to support the CHC application as robustly as possible
• To work alongside NHS Berks CHC team and associated outsourced CHS agency, and be confident to challenge as necessary to ensure the best possible outcomes for the Council are achieved
• To complete Local Authority review documents assessments and reviews to support the CHC process
• To participate in Health led CHC MDT – Multi Disciplinary Team meetings
• Knowledge of Care Director would be preferable
• To liaise with families and carers to explain the process
• To maintain case records and to fulfil Service procedures and statutory duties
• To maintain an up to date spreadsheet of all referrals, actions and progress on each individual
