There is a vacancy for a full time qualified Social Worker, within the Adult All Age Disability Team Social Work team.

This post is for a Transition Social Worker to work with those who have a physical disability and / or autism. This is a newly created post due to the move to the All Age Disability team and the successful candidate will have the Transition Social Workers in the Learning Disability Team for support.

The All Age Disability teams work with Service Users and their families to maximise independence and opportunities which enable service users to enjoy purposeful, meaningful and fulfilled lives. The candidate must have experience in working in transition within learning disability or physical disability teams. Experience of transition, assessment, care planning, person centred planning and care management are essential, as is the ability to use IT in a competent manner and to meet performance standards. We are looking for an enthusiastic individual with a strong value base and committed to strength based practice
