Leaving Care Support Worker - Children’s Services - Bolton 

Are you a passionate, dedicated Leaving Care Support Worker looking for your next role? 4Recruitment Services are recruiting a Leaving Care Support Worker to join a Leaving Care Team based in Bolton. 

Umbrella Rate: £21.21 per hour 

Duration: 3 months initially

Hours: Monday to Friday, 9am – 5pm, 37 hrs/week – Hybrid 

Leaving Care Support Worker role: Full JD Available

  • Direct work with Looked After Children, Care Leavers and Homeless Young People aged 16 – 25, developing their pathway into independence
  • Take responsibility on a day-to-day basis for your individual caseload
  • Home/ Community visits
  • Support to improve life chances and outcomes
  • Offer assessment and support to young homeless people
  • Support to access accommodation, employment, education 
  • Support with independent living skills such as budgeting, cooking, cleaning
  • Assess and implement plans to address behaviour
  • Assess their individual needs with a course of action
  • Prepare, produce, implement and evaluate plans
  • Prepare for and participate in decision making forums
  • Work with partner agencies and colleagues to provide appropriate services
  • Maintain appropriate records of services provided to service users
  • Attend reviews, case conferences, pathway planning and meetings where appropriate

Essential Requirements

  • UK Driving Licence, access to a vehicle and business insurance
  • Recent extensive experience working with young people leaving care
  • Ability to undertake assessments and review programmes, including the provision of pathway plans
  • Knowledge of issues facing young people leaving care or becoming homeless
  • Ability to engage with resistant Young People
  • Ability to visit clients in the Bolton area 
  • Excellent written and verbal communication skills
  • You Must Have Right to Live & Work in the UK
  • Enhanced DBS registered to the update service
