Job Description

We are currently looking for a Senior Social Worker/Social Worker - Young Adult's Team. You will be working in Cambridge for the council.

Pay rates up to £32ph

Job Purpose

The Young Adults Team (YAT) is a boroughwide team, which supports young people who it is felt are likely to require support from adult social care once they reach the age of 18. The team is part of the wider Adult Social Care Team within the council.

The Young Adults Team (YAT) works alongside young people aged 14 to 25, their family/carers carrying out outcome focused assessments and developing support plans to meet the young adults assessed eligible needs as defined in the Care Act 2014 that promotes independence and meets identified outcomes and aspirations

Support could include; advice and information and signposting, receiving a direct payment, assistive technology, support to help the young person to remain living in their family/own home, supported living, access to employment, day-time opportunities and residential and nursing home placements
