Adult's Social Worker - Review Team

We are currently looking for a Adult's Social Worker - Review Team to work with one of our clients based in Southend. You will be working for the council.

Monday - Friday 

Shift Times may vary

Perm contract 

Salary paying up to  £37,890 per year

Job Purpose

You will support the Team by undertaking Annual Reviews of the Commissioned Services for Adults that are supported by the Local Authority in and at times outside the Southend Borough. The role is responsible for ensuring the safety and well being of the people and to give consideration to Continuing Health Care eligibility. The Reviews must be person centred ensuring that the views of the person and their carers / advocates are heard. The approach must be strength based at all times utilising the person's personal and community Assets
