CPN Band 6- Recovery & Wellbeing Team - Essex

Are you a passionate, dedicated CPN looking for your next role? 4Recruitment Services are recruiting for Band 6 CPN's to work with the Recovery & Wellbeing Team in and around Essex.

Pay Rate: £20.28-£23.28 per hour

What you will be doing:

  • To provide a wide range of high quality community care services which will enable people to lead as independent a life as possible, and have meaningful choices.
  • To establish and maintain effective individualised care using the Care Programme Approach (CPA) by:-
  1. Undertaking assessment of risk and formulating care plans to manage and    minimise risk.
  2. Planning, implementing and reviewing interventions designed to meet assessed needs.
  3. Providing advice and information to service users, their carers and their families in order to alleviate distress and work in partnership, as far as possible, with all the available support systems.
  4. Providing clear, accurate and concise documentation of the service users care as required by the Trusts Care Programme Approach (CPA) Policy.
  • To participate in the initial assessment and selection of potential clients ensuring that their physical/psychological, nursing and social care needs are taken into account at all times.
  • To participate in team meetings
  • Seek to establish a therapeutic relationship with individual clients and their families.
  • Ensure effective and confidential communication between all care professionals.
  • Provide significant data governing client care and treatment by:-
  1. Participating in informal or formal case conferences and discussions.
  2. Maintaining a secure system of record keeping and information exchange
  3. Establishing regular written reviews of progress
  4. Ensure that information is recorded on the client information database.
  5. To develop close links with relatives/carers and ensure that effective communication takes place especially when clients are admitted or being prepared for discharge from hospital and that all appropriate agencies are informed, so that follow up care can take place.
  6. To advise the manager and Professional Nursing Advisor on all breaches of nursing professional conduct as required by the Nursing and Midwifery Council’s Code of Professional Conduct.

Essential Requirements:

  • Must Have a Valid NMC Pin
  • Must  have the Ability to advocate on behalf of clients and carers.
  • Detailed understanding and knowledge of mental Health practice and procedure policy.
  • It is essential for this post that you are able to drive, have a full driving licence, and have a car available for your use
