We are currently looking for a Adult's Team Manager - Integrated Discharge Team to work with one of our clients based in Rotherham. You will be working for the council.

Job Purpose

An exciting opportunity, as a Team Manager is available for up to 6 months in the Integrated Discharge Team based at Rotherham Hospital. This offers an excellent opportunity to work in partnership with health colleagues and the wider multi-disciplinary team, something which Rotherham pride themselves on executing with excellence.

This post is to support the day to day management of the team, allocating work to ensure safe and timely discharges of patients from the acute hospital trust. You will support to authorise Care Act assessments and support plans and have oversight of the workload in all areas of the team, including from the acute hospital, non acute bed bases and reviews in community.

The team is an integrated health and social care team made up of social care professionals, nurses and therapy staff. You will have direct responsibility for the social care staff within the team, working directly alongside the IDT Nurse Lead.

The team follow a “home first” ethos, working to a discharge to assess model over a 7 day period. The team are currently following the COVID-19 Hospital Discharge Guidance issued by the government in relation to the COVID-19 response.
