About the Role: As an Advanced Adults Social Worker, you will play a pivotal role in our Single Point of Access Team – the first point of contact for triage and screening. Your responsibilities will include providing timely advice, information, and expert triage of referrals received for adult care and community health services. Additionally, you will be involved in community-based care management clinics, offering a holistic approach to client well-being.

Key Requirements:
- Qualified Social Worker: Hold a relevant social work qualification and be registered with Social Work England.
- Communication Skills: Excellent interpersonal and communication skills to engage effectively with clients and multidisciplinary teams.
- ⚖️ Assessment and Triage: Experience in timely assessment and triage of adult care referrals.
- ️ ICT Proficiency: Comfortable with remote working and the ability to collect ICT equipment from the Town Hall in Weston Super Mare during the induction phase.
