Health and Social Care Officer - Nottingham
Are you a passionate, dedicated Health and Social Care Officer looking for your next role? 4Recruitment Services are recruiting a Health and Social Care Officer to join an Adult Social Care Team based in Nottingham.
Umbrella Pay- £17.10 per hour.
9 months initially.
9am – 5pm, Monday – Friday.
37 hours a week.
The role:
- Help provide a streamlined and effective referral service for vulnerable adults, often dealing with complex needs and challenging or unpredictable situations.
- Offer guidance, perform triage, and coordinate information to facilitate decision-making and risk management for citizens seeking or needing access to social care and health services.
- Manage telephone calls, inquiries, and referrals from citizens, vulnerable adults, families, and caregivers in an efficient, timely, and customer-focused manner.
- Provide a consistent level of customer service that reflects our vision, values, aims, and objectives.
- Handle specialized or escalated calls from Health and Social Care Support Coordinators, Health and Social Care Support Advisors, or other colleagues.
- Possess knowledge and understanding of the various relevant services and their referral criteria within social care and health, enabling the provision of appropriate advice and guidance, and ensuring this information is kept up to date.
- Collaborate with relevant clinicians, practitioners, and health and social care providers to coordinate appropriate care packages and ongoing care needs. This includes triaging and directing citizens to other services, tracking, and monitoring the pickup and handover of clients to the crisis response team.
- Make decisions on referrals and requests concerning complex situations, safeguarding concerns, and risk management by triaging and coordinating to ensure clients are seen by the most appropriate team or service at the right time to meet their needs.
- maintain the confidentiality and sensitivity of customer inquiries.
- Participate in team, staff, and multidisciplinary meetings as needed, engaging in case discussions, team activities, and individual supervision sessions with the line manager according to the service area's supervision policy.
- Receive referrals and arrange the most suitable service for the citizen's needs, considering available capacity within the required response/assessment timeframe.
- Additional tasks and responsibilities as needed.
Requirements
- Extensive experience in Adult Social Care referrals and assessments.
- Proficient understanding of Microsoft Office and strong keyboard proficiency.
- Advanced organizational, administrative, and negotiation skills, coupled with a comprehensive comprehension of customer care matters.
- Exceptional communication abilities, both verbal and written, with proficiency in handling potentially aggressive, emotional, and challenging behaviors, whether over the phone or in person, including active listening skills.
- 5 GCSEs or equivalent, with English and Maths grades C and above.
- Enhanced DBS registered to the update service.
