Calderdale Council - Support Co-ordinator
Location: The Acorns, West Yorkshire, HD6 3YU
Contract: Initial One Month Contract with View to Extensions
Hours: 9:00am – 5:00pm / Monday to Friday
Payrate: £10.23 an Hour
Role Purpose:
Contribute and assist individuals to be involved in their assessments and to help them make decisions based upon their assessed needs.
Produce high quality support plans and guidelines which draw on the strengths of individuals and include creative and informal solutions to assessed needs where possible.
Make decisions that are in accordance with relevant policies guidance and deploy the correct use of legislation aimed at maintaining the independence of vulnerable adults and assist them to maximise their own abilities and networks of support
Principal duties and responsibilities
All staff are expected to maintain high standards of customer care in the context of the council’s Core Values, to uphold the Equality and Diversity Policy and health and safety standards and to participate in training activities necessary to their post.

Provide high quality support to individuals who receive a service including direct support. This involves co-ordinating a schedule of activities, staff resources and transport.
Provide opportunities to enable individuals to acquire or develop the skills needed to maximise their independence and to lead a fulfilling life by providing appropriate information, opportunity and support.
Utilise, monitor and update recording systems to ensure you and others provide appropriate support at all times to individuals who use the service. Ensure that documented information is accurate, up-to-date and reflective of the service involvement with an individual.
Support individuals to meet both their personal financial resources and personal care needs sensitively and appropriately maintaining the appropriate records.
Support individuals to take prescribed medication, complying with all medication policies and procedures – including storage, administering and record keeping.
Support individuals to identify and choose their support needs to include devising, implementing, maintaining and reviewing support plans and guidelines and ensure they accurately reflect the individual’s needs.
Ensure that any issues of significant risk and/or complexity are reported to an Assistant Manager. Complete risk assessments in line with Council policy.
Actively promote the professional development of individual team members through regular supervision, identifying appropriate training opportunities.
Ensure the service is adequately covered in staff absence and to take responsibility, as required, for covering the work of the assistant manager in their absence.
Support an individual to liaise with other professionals in statutory and non-statutory organisations to establish good community links with voluntary agencies to ensure better outcomes for the person and to help them to maintain their independence, autonomy, choice and control.
Responsibility for the building, equipment and external areas ensuring they are maintained in a safe and satisfactory condition in compliance with Health and Safety regulations including undertaking monthly and annual audits.
