Team Leader - Hospital Discharge - Halifax

Are you a passionate, dedicated Team Leader looking for your next role? 4Recruitment Services are recruiting a Team Leader to join a Hospital Discharge Team based in Halifax.

Umbrella Pay - £40 per hour.

6 months contract initially.

30 hours a week – Hybrid (must be able to attend the hospital at least 2-3 days per week)

The role:

  • Assist the hospital discharge team and Intermediate Care team in ensuring safe and timely discharges.
  • Offer professional support to the team and assist the Manager in coordinating, deploying, and managing resources, including staffing resources, effectively.
  • Ensure effective systems are established to screen referrals against eligibility criteria. Collaborate with partners to manage referrals through resourceful deployment of staffing and other available options.
  • Prioritize and assign tasks to suitable staff members based on the complexity and urgency of service requests. Utilize the collective resources and skills of the entire team and other capacities within the service.
  • Responsible for ensuring that the review and assessment of Support Plans adhere to practice guidelines and making necessary adjustments to reflect evolving needs.
  • Support the Team Manager in ensuring adequate coverage of the team's work during staff absences. Take on responsibilities, as needed, to cover for the Team Manager during their absence and oversee/manage sickness absence in compliance with Council policies.
  • Ensure efficient management and allocation of resources within budgetary constraints to achieve agreed policy objectives and service priorities. Communicate effectively with the team regarding budget management strategies.
  • Collaborate with the Team Manager to establish a performance development profile and personalized training and development plan for individuals, including professionally qualified staff.

Requirements

  • Professional qualification in social work (existing staff within Fieldwork Teams may include a professional nursing qualification).
  • Extensive familiarity with Adult Social Care/NHS legislation and policy guidance, and understanding of the principles, practices, and provisions of community care.
  • Capability to manage complex and challenging situations, including identification and management of risks, particularly risks to vulnerable adults.
  • Capability to carry out assigned responsibilities from the Team Manager and act in their absence.
  • Extensive experience working with older people, adults with physical disabilities, or individuals with learning disabilities.
  • Experience in care management.
  • Enhanced DBS registered to the update service.
