Pay Rates: £14.00 - £18.00  Per Hour

What the roles of a Brokerage Officer: Full JD Available

This is a Continuing Healthcare post, based within the Integrated Commissioning Team that works across health and social care.

The role is to:

Broker cost effective and timely services for vulnerable patients on behalf of the councils Continuing Healthcare Team (South East London CCG)

Lead the process for securing high quality, timely and cost-effective care packages of care for eligible continuing healthcare patients.

Build effective relationships with relevant professionals and key stakeholders (i.e. patients and their families, nurse assessors, local authorities, hospital discharge teams, placement providers)

Essential Requirements:

• Experience of brokering services
• Experience of dealing with consumers or other customer services.
• Experience of dealing with people at all levels
• Ability to understand care packages and cost structure and proven track record to formulate strategies to achieve value for money
• Ability to understand budgetary information
• Proven ability of working in a team environment and delivering team objectives.
• Experience of NHS or Social Services working environment.
• Experience of working within NHS Continuing Healthcare
• You Must Have Right to Live & Work in the UK
