4Recruitment Services require an experienced Social Worker to work within a Adults learing disability team based within Bolton. The Successful Social Worker will be required to have a minimum of 2 years recent Social Work experience of working within a Adults Learning Disabilty team.

The successful Social Worker will have experience of:

• • Carrying out full social care reassessment making appropriate referrals for specialist assessments
  • Intergrated working with LD nurses, OT, reablement team
  • Working with family members keeping them informed and on-side
  • Conducting capacity assessments and arranging best interest meetings
  • Presenting cases to panel for change package care
  • Reviewing Support Plans, regularly and targets set to enable service users to progress and move on within the community
  • Evaluating how well current needs are being met in relation to the Care Act
  • Identifying potential development and specify how that will be met
  • Negotiating placement provision or costs where appropriate (with commissioning support)
  • Identifying whether placement is best value for money, Identifying cost/benefits for any interventions
  • Identify other funding mechanisms for placement

To apply for this the Social Worker must:

• Have a relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent)
• Be HCPC Registered
• Be eligible to work within the UK

The pay rate for this role is £25.00 per hour on a 3 month locum basis
