Brokerage Officer – Neath (Temp Vacancy)

Pay Rate: £11.00ph

Full-Time (37 hours)

Job Purpose:

  • To purchase social care provision across Neath Port Talbot on behalf of Adult Services.
  • To be responsible for arranging appropriate packages of care for individual service users as specified by care managers.
  • To provide a single point of contact for the purchase of social care services and across Neath Port Talbot, liaising with Care Management Teams, Business Support, and internal and external providers, including advising and liaising with providers who experience difficulties in implementing care packages.
  • To support the Commissioning Unit to develop a high quality, reliable, sustainable and timely care market by coordinating placements and setting up individual placement contracts.
  • To respond to and support Care Management staff to access packages of care through the brokerage system.
  • To support the development and improvement of social care services and as such be flexible in the role, consistent with the purposes and grade of the post.

Accountabilities:  

  • To receive and access individual care plans from the Care Managers in order to negotiate and co-ordinate provision, for social care services when required to meet the needs of service users in Neath Port Talbot CBC
  • To receive and access risk assessments, Medicine Management and moving and handling assessments to ensure that providers are in receipt of the required documentation before packages of care commenced.
  • To make timely arrangements based on even handed judgements about the type of service, effectiveness, quality and value for money across statutory, voluntary, private and out of county provision in order to meet the needs of service users in Neath Port Talbot.
  • To prioritise the workload, consistent with the purposes of the post, to ensure that emergency referrals and increases are dealt with as a priority in order to ensure service users’ safety in their own homes or avoid inappropriate hospital admission or delayed discharges.
  • To support Care Management Staff to access care packages through the brokerage system. 
  • To use the brokering system to ensure best value for money with providers to reduce costs.
  • To issue individual service agreements to service providers ensuring financial accuracy so that the Financial Assessment Team can process invoices in an efficient and timely manner.
  • To update the Oracle system as required ensuring providers are paid appropriately for the provision care including when care packages are changed according to need.
  • To input data on services arranged, the costs and intended outcomes into systems and ensure the information is passed to the Social Work and Care Management Team.
  • To comply with paper and IT systems to enable the Council and organisations to provide information on capacity and current provision; to refer appropriately where deficits in performance are identified; to provide statistical and supporting information on services when required; and to retain details of unmet need for commissioning and contracting purposes.
  • Attend meetings consistent with the purposes of the post and as requested by your line manager.
  • To undertake any other duties that may be reasonably required by the Commissioning Officer commensurate with the role, responsibility, level and grading of the post.
  • To ensure compliance with the Data Protection Act.
